COMMUNITY ASSESSMENTS

Identify the health and resource needs of your members — especially issues that you can’t detect on a phone call

In-person assessments are the most effective way to engage, educate and understand your members.
Matrix Medical Network gives you unparalleled insight and helps you connect directly to your members via our Community Assessments

MEETING REQUIREMENTS

Centers for Medicare & Medicaid Services and each state Medicaid program have the same requirements: every newly enrolled member must receive an initial assessment and an annual reassessment. However, health plans can be challenged to get these done on a timely basis.
This is where Matrix comes in.

A QUALITY MEMBER EXPERIENCE

Matrix Community Assessment Services provide in-person member assessments that are 100 percent compliant with CMS and state-based regulatory requirements. After engaging, educating and assessing nearly a half-million plan members – 95% say they are happy with their Matrix experience.

The Matrix Community Assessment Services platform is supported by our:

• NCQA certification in health appraisals
• Digital data assessment technologies for easy and secure information transfer
• Diverse network of community-based nurse practitioners, nurses, certified nursing assistants and social workers
• Delegated entity contract status for state Medicaid demonstrations

We quickly engage with your eligible population and provide you with supporting documentation for managing your “total-population-reached” compliance requirements.

MEETING YOUR MEMBERS' NEEDS

The Matrix in-person visit usually takes about 60 minutes and provides a deep understanding of your members. We help address lower-risk needs through education, member goal setting and creating initial plans of care. Higher-risk members benefit from access to additional programs and services through resource and care management referrals.

Our comprehensive assessments are tuned to state-specific requirements and provide you with real-time member insights on matters, including:

When visiting renewing members, our providers have access to the information from the prior visit(s).
