DIRECTLY ADDRESS MULTIPLE QUALITY MEASURES

Matrix addresses multiple care gaps during a single quality visit including:

Bone Density

  • Bone density testing is used to screen for osteoporosis or a weakening of the bones.
  • The test is done with a simple ultrasound heel scan on the mobile health clinic (MHC) or in the home with a similar ultrasound of the arm.

Diabetic Retinal Exam

  • Diabetic retinopathy is the leading cause of blindness in adults 20-64 years of age and occurs in 80% of people who have had diabetes for more than 20 years.
  • Matrix conducts a retinal eye exam, utilizing a state-of-the-art digital imager to capture images of the retina for review and diagnosis by a board-certified retinal specialist.

Fecal Immunochemical Test (FIT)

  • Colon and rectum cancers are the second leading cause of cancer death in the United States. The Fecal Immunochemical Test is a screening for colorectal cancer which tests for hidden blood in the stool – an early sign of cancer.
  • Matrix provides a FIT kit then follows up with the member to ensure the test is completed.

Influenza (Flu) Vaccine

  • Annual flu vaccines are the best way to lower the risk and spread of the flu in communities.
  • Matrix provides yearly immunizations to protect against seasonal influenza. (MHC only)

Hemoglobin A1c Test

  • Measuring glycol-hemoglobin (HbA1c) reflects the average blood sugar levels over 2-3 months. The higher the HbA1c, the greater the risk of developing diabetes-related complications.
  • Matrix collects a blood sample for lab testing to show blood sugar level over the past 2-3 months.

Mammography

  • A mammogram is used as a screening tool to detect for signs of breast cancer.
  • Matrix conducts digital mammograms with a low-dose x-ray of the breast. (MHC only)

Microalbumin

  • Urine microalbumin level refers to small amounts of albumin, a serum protein that may be found in the urine. This can be an early sign of diabetic kidney damage.
  • Matrix collects a urine sample for lab testing to detect signs of kidney disease.

Future Quality Measures

  • As an industry leader, Matrix continually monitors HEDIS/Stars measures updates and/or enhancements, adding appropriate screenings and tests as they relate to quality.
