Matrix Medical Network provides convenient in-home well child visits for infants and children up to 21 years of age. Being in the home allows us to gather deep insights beyond what is possible in a typical clinical setting.
We help connect the 51% of children (ages 1 – 17) nationally without a medical home* to a care delivery system of primary care and specialty physicians, care management programs, local support resources and emergency services as needed.
While in the home, we can evaluate each child’s medical history, perform a thorough age appropriate exam to assess their current health, and identify health and development issues early before they pose greater risks later in life.

Each visit we perform closes gaps in care and directly addresses eight pediatric HEDIS measures. We adhere to the National Committee for Quality Assurance’s Early and Periodic Screening, Diagnostic and Treatment program to help you meet established goals.
Being in the home allows us to gather deep insights into a child’s living situation and potential environmental risks.
We identify social, environmental and financial issues that may inhibit a family’s efforts to improve or maintain the health of their children, and help them get the necessary support.
